PSindex™ Report Update

Your Team Survey report now includes four numbers: 4 Stages score, Blue Zone score, Neutral Zone score, and Red Zone score.

We're excited to publish the updated Team Survey report for 4 Stages: Measure™! We're confident that this update will help you interpret 4 Stages Team Survey data with greater clarity and provide an even more actionable report. The survey instrument itself has not changed. Please reference the Team Survey Technical Report for information regarding the survey instrument itself.
 
What's new?
Your Team Survey report now includes four numbers: 4 Stages score, Blue Zone score, Neutral Zone score, and Red Zone score as shown below:
 
4 Stages Score:
  • This overall score is determined by a team's Blue Zone score less that team's Red Zone score. For instance, if a team's Blue Zone score is 16 and its Red Zone score is 11, its overall score will be 5.
  • This overall score looks at all 12 survey items to assess the total psychological safety of a team.
  • Possible scores range from -100 to 100. This increased possible range results in greater score spread for most organizations, making it easier to identify those teams that are healthy and those that are unhealthy.
 
 
Blue Zone score:
  • Blue Zone scores represent the percentage of respondents who selected 9 or 10.
  • Blue Zones are areas of rewarded vulnerability.
  • When individuals are in a Blue Zone they tend to offer a Performance Response which leads to value creation.
  • Rewarded vulnerability:
    • Activates the pleasure/reward centers of the brain.
    • Arrests the self-censoring instinct.
    • Releases discretionary effort.
 
Neutral Zone score:
  • Neutral Zone scores represent the percentage of respondents who selected 7 or 8.
  • Neutral zones are areas of relatively infrequent rewarded vulnerability and relatively frequent punished vulnerability.
  • When individuals are in a neutral zone they often feel role-based security to fulfill their responsibilities but do not feel enough support to take bigger risks or give more discretionary effort.
 
Red Zone score:
  • Red Zone scores represent the percentage of respondents who selected 0-6.
  • Red Zones are environments of punished vulnerability.
  • When individuals are in a Red Zone they offer a Fear Response which leads to acts of self-preservation and pain avoidance.
  • Punished Vulnerability:
    • Activates the pain centers of the brain.
    • Triggers the self-censoring instinct.
    • Shifts the individual to a defensive mode of performance.
 
Percentiles:
  • Percentiles help you understand how a team scored relative to The 4 Stages Team Survey global database with over 200k responses. There are no benchmarks by geography or industry at this time.
  • Percentiles are provided for the following: 4 Stages Score, each stage of psychological safety, and each survey item.
  • If a team's percentile is 36, this team scored higher than 36% of all teams and lower than 64% of all teams. 
Survey Frequency:
  • Psychological safety is delicate and dynamic. It requires constant reinforcement. The job is never done!
  • LeaderFactor recommends administering the Team Survey once each quarter.
 
FAQs:
How do I share my Team Survey results?
  • Select 'Share Results With Manager'. By selecting this button, the manager of the team will receive an email notifying them that they can access Team Survey results in the LeaderFactor.app account.
  • Select "Download Report" under Options to download a copy of the Team Survey results.
How does this change affect historical surveys?
  • This reporting update does not alter the data of historical surveys. Any surveys completed in the past will inherit the new report structure.
