1. Go to the Trainings tab in the left-hand sidebar, then click Training Admin.
2. Click New Cohort.
3. Click EQindex Self Assessment.
4. In the Setup window, navigate to the Activity section and click Add Participants.
5. In the popup, you can either enter participant information manually or download the template.

If your survey group has fewer than 3 participants, manually enter the rows. Once you've done so proceed to the next instruction.

If your group includes 3 or more participants, click Download Template to upload them in bulk.
6. Click Download Template to add participants to your survey.
7. Remove any example data in the downloaded file and enter your participant information in the appropriate fields.
8. After entering all participant information into the sheet, select and copy the entire dataset.
9. Navigate back to the LeaderFactor app and proceed to Step 2 by clicking Next.
10. Paste the copied participant information into the text box in Step 2. Click Next.
11. Ensure that your provided information matches with the columns from the template. Click Import.
11. You’ll now see a list of all your participants. If needed, you can edit their information directly in this view. To remove a participant, click the – button on the right side of their row. To add more participants, click Add Participants in the top-right corner.
12. Once your participant information is finalized, click Import Spreadsheet in the bottom-right corner. You'll be redirected to the set up page where you can proceed to Schedule Survey.
13. The next window will display how many participants are enrolled in the EQindex assessment, how many have started it, and how many have submitted their action plans.
14. Complete the remaining fields on the assessment setup page:

Cohort Name: Enter a name for your group of participants.

Invitation Note: Add a custom message to be included in the invitation email.

Enrollment Starts: Choose the date you’d like the assessment to be sent to participants.

Location: Optionally add a location if you have an upcoming training tied to a specific place.

Terms and Pricing: Confirm your agreement by toggling on "I Agree to the Terms and Pricing."
15. Preview the invitation email to ensure all details are correct.
16. Once you have completed all fields on the survey scheduling page, click Save.
17. Your assessment is now scheduled to launch.
18. At any given time, you can navigate to the Participants tab to monitor assessment progress.
